Reforming only works if you are part of the same culture/group as the previous country, it doesen't exist, and you might need to own it's old capital orleons, Athens, Northumbria (has to be realeased first), and Styria (ditto northumbria) are some good examples of countries that can reform. You forgot Persia, Mughals, Westphilia, Hannover, Sardinia Piedmont, Romania, and reforming France, Byzantium, England, and Austria.
